Verzenio (abemaciclib) is an oral targeted cancer therapy known as a CDK4/6 inhibitor. It is used to treat hormone receptor-positive, HER2-negative breast cancer, both in advanced disease and in certain high-risk early breast cancers, usually combined with hormone therapy. It works by blocking proteins that drive cancer cell division.

How is this medication used?

Take the prescribed dose by mouth twice daily, approximately 12 hours apart, with or without food, continuously without breaks. Swallow tablets whole and do not take a replacement dose if you vomit; simply take the next scheduled dose. Continue as directed by your doctor.

What are the possible side effects?

The most common side effect is diarrhea, which usually starts early in treatment and is managed with fluids and antidiarrheal medication. Other side effects include fatigue, nausea, low white blood cell counts, infections, and hair thinning. Blood tests are needed to monitor blood counts and liver function; rarely, blood clots and lung inflammation can occur.

What are the potential interactions?

Avoid grapefruit and strong CYP3A4 inhibitors (ketoconazole, clarithromycin), which raise abemaciclib levels, and strong inducers (rifampicin, St. John's Wort), which lower them. Tell your doctor about all medications and supplements you take.
